365 offers a variety of Habilitation Services which are designed to provide home and community support for individuals with intellectual and developmental disabilities. Through these services, our goal is to help individuals achieve and maintain an everyday life rooted in connection to their community.
Our Approach
At 365, our trained Direct Support Professionals provide one on one care focused on person centered practices that promote life skills training, access to the community and maintaining independence. We dedicate ourselves to working together throughout the ISP process and ensuring that measurable goals are in place for success.

Skilled Needs Funded Through the ID Waiver Program
Our highly trained and supported nurses manage patient care and promote the overall health of the Individual living in the community. With a focus on providing one-on-one care tailored to a individual’s unique health challenges, we also help the individual and their family maintain the best possible quality of life. Maximizing the patient's ability to access their normal activities and ensuring they can remain safely in their own home is a top priority for our team. Our nurses initiate and manage the physician-directed interventions for individuals with complex medical needs including total care of their person, ventilator management, Tracheostomy care, supplemental feedings, Wound care, and medication administration. Living at home can increase an individual's overall quality of life and improve their health-our nurses are there to make that mission a reality for the individual’s that we serve.
